Initial Consultation

Your initial appointment will consist of a new patient exam to assess your concerns and condition(s). We will then discuss a diagnosis and develop treatment options. Occasionally, treatment can be performed the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.

Please assist us by providing the following information in advance of your consultation:

  • Your referral slip and any dental Panoramic X-rays, dental CBCT image, CT scans, MRI films,  or Sleep studies (if applicable)
  • A list of your current medical prescriptions and which conditions they are being used for
  • Please complete the online form prior to your visit. We review the information provided so we can best serve you during your appointment.
  • Based on the findings from the initial visit, you may need additional diagnostic tests. We will share our findings with you and develop a treatment plan that works best for you individual needs.

IMPORTANT: A parent or guardian must accompany all patients under 18 at the consultation visit.

X-Ray/CT/MRI

If your physician or dentist has ordered a previous diagnostic study, you may request to have the films and results forwarded to our office. If there is not enough time, please pick them up and bring them to your initial consultation.
